After Applying
Once your application is received and processed, you will receive written notification of your enrollment status in the VA health care system.
If you do not hear back within a couple of months, we do recommend calling the Springfield CBOC to check the status of your application: 937-342-5480.
WHEN COMING TO OUR OFFICE
Please bring the following documents:
- Card for any other health insurance you are are receiving
- Discharge or separation papers (DD214 or equivalent)
- Annual income for the prior year (Veteran and spouse if applicable)
- Proof of payment for medical, funeral and/or education expenses
